Role Description The Transitional Housing Case Manager is a full-time, on-site role based in Newmarket, ON. This position is responsible for supporting residents in transitional housing through individualized case management, including intake assessments, goal setting, and ongoing progress reviews.

The Case

Manager coordinates services and referrals to community resources such as employment programs, mental health supports, addiction services, and housing agencies. Daily tasks include maintaining accurate documentation, developing and updating care plans, advocating on behalf of residents, and facilitating life skills or support groups. The role also involves collaborating with internal staff and external partners,



monitoring program compliance, and contributing to a safe, respectful, and inclusive workplace for all residents.

Qualifications

- Demonstrated case management and counseling skills, including assessment, goal planning, and crisis intervention.
- Experience working with individuals facing homelessness, mental health challenges, substance use, or other complex social issues.
- Strong communication and interpersonal skills, with the ability to build trust and maintain professional boundaries.
- Organizational and documentation skills, including accurate record-keeping and familiarity with basic computer applications.
- Knowledge of community resources, social services, and housing supports within the Newmarket and surrounding area.
- Ability to work collaboratively in a multidisciplinary team and develop effective partnerships with external agencies.
- Post-secondary education in Social Work, Social Services, Psychology, or a related human services field (or equivalent combination of education and experience).
- Commitment to equity, diversity, inclusion, and trauma-informed, person-centered practice.
- Ability to work on-site with flexible schedule as needed, and to respond calmly and safely in challenging situations.
