Support community integration as a dedicated Community Inclusion Support Worker in a full time capacity. Assist adults with developmental disabilities in achieving independence and participating in community life through tailored support.

Reporting to the Community Inclusion & Outreach Manager, this role emphasizes a respectful, culturally grounded approach. The position involves working closely with families, caregivers, and multidisciplinary teams to ensure coordinated service delivery. Responsibilities focus on daily living assistance, life skills development, and fostering community connections.

Key Responsibilities:
• Promote individual choice and participation in life decisions
• Maintain daily logs and case management alongside the CLBC Coordinator




• Foster supportive relationships that encourage self-determination
• Help individuals with health, hygiene, and budgeting tasks
• Communicate service needs effectively with families and providers

Requirements:
• High school diploma or equivalent
• 1–2 years in community support services
• Valid First Aid and CPR certification
• Proficiency in Microsoft Office
• Clear Criminal Record Check including Vulnerable Sector
