Key Responsibilities:
• Create effective Care Plans in collaboration with health services
• Deliver nursing care directly to patients in various settings
• Administer IV therapy, hydration management, and wound care
• Maintain communication with Ontario Health atHome
• Complete other clinical duties as required

Requirements:
• Registered Practical Nurse degree or diploma
• Current CNO registration
• Commitment to patient-centered care
• Strong English communication skills
• Background check and TB test required
