You will work in the HART Hub program, which is dedicated to providing comprehensive resources to individuals dealing with addiction and mental health challenges. This dynamic role involves traveling across Waterloo Region, primarily in Cambridge, to foster engaging relationships with participants that empower them toward recovery goals. You will help create individual support plans and collaborate with multi-disciplinary teams.
Key Responsibilities:
• Engage participants and build meaningful relationships
• Ensure harm reduction and primary care education
• Develop personalized support plans with assessment tools
• Collaborate with community partners for resource connection
• Participate in case conferences for holistic support
Requirements:
• Related diploma or degree required
• 2+ years in case management or planning
• Knowledge of harm reduction strategies
• Solid communication and problem-solving skills
• Ongoing commitment to professional training
