Join Extendicare as a Part time Registered Practical Nurse (RPN) dedicated to patient care. Provide effective nursing services in accordance with provincial care standards and enhance residents' quality of life.

In this role, you'll be responsible for planning and delivering patient-centered care while collaborating with an interprofessional team. You will conduct thorough assessments, implement care plans, and ensure accurate documentation and communication regarding patient status. Your input will also help coordinate care conferences and respond to residents’ changing conditions.

Key Responsibilities:
• Assess and document residents' clinical needs
• Implement and evaluate care/service plans




• Provide medication and treatment administration
• Teach and supervise unregulated care providers
• Collaborate with the care team on patient transfers

Requirements:
• Registered as a Practical Nurse in the province
• Experience with seniors and unregulated care providers
• Excellent critical thinking and time management skills
• Current Basic Cardiac Life Support certification
• Vulnerable Sector Screening within last six months
