As a Community Resource Navigator at PACE, you will collaborate with interdisciplinary teams, caregivers, and clients to foster well-being and successful transitions. This role emphasizes facilitating independent living, advocating for clients, and promoting access to essential community resources. Your efforts will significantly impact the lives of those navigating complex social services.
Key Responsibilities:
• Facilitate client planning for personalized goals
• Assist with community resource navigation and development
• Coordinate care transitions between service settings
• Provide client education on available services
• Support program quality and data collection initiatives
Requirements:
• Degree in Social Work or related field
• 3 to 5 years in social services or health support
• Experience in case management and community navigation
• Knowledge of relevant standards and practices
• Solid verbal and organizational skills
