As part of the Housing, Facilities, and Employment Services team, you will engage with clients facing serious mental health challenges. Your role involves collaboratively developing care plans, advocating for housing solutions, and educating clients on symptom management. You will work closely with community landlords to promote sustainability and independence.

Key Responsibilities:
• Collaborate with clients and resources to address housing needs
• Educate clients on services and protocols
• Create and evaluate relapse prevention and wellness plans




• Implement treatment plans prioritizing recovery and independence
• Support family involvement in the recovery process

Requirements:
• Minimum of a Bachelor’s degree in a relevant field
• Two years’ experience working with youth in housing support
• Comprehensive understanding of mental health legislation
• Robust computer skills with Microsoft applications
• Ability to provide short-term crisis support
