This role focuses on overseeing clinic intakes and triage assessments while prioritizing client needs and ensuring smooth transitions to primary care. You will work closely with healthcare partners and community agencies to enhance access to health services for diverse populations facing barriers.

Key Responsibilities:
• Manage and supervise primary care intake processes
• Prioritize client intakes based on urgency
• Ensure compliance with documentation standards
• Collaborate with clinicians and administrative teams
• Track referral follow-ups for service initiation

Requirements:
• Valid Nurse registration in Ontario
• Two years' experience in primary care or community settings
• Knowledge of social determinants impacting health
• Proven communication skills in English, a second language is an asset
• Ability to maintain confidentiality and privacy standards
