Support vulnerable community members as a Part time Community Worker with Pinecrest-Queensway Community Health Centre. This role involves client-centered assistance and outreach activities in low-income neighborhoods.

The Community Worker – Wrap Around Support (WAS) provides essential services within community development. You will work collaboratively with clients and partner organizations to connect individuals to vital resources and produce holistic support. This temporary position reports to the Community House Coordinator and emphasizes outreach, client well-being, and resource navigation.

Key Responsibilities:
• Conduct outreach to identify community needs
• Provide client support and resource linkages




• Assist with client assessment and referrals
• Foster ongoing community engagement and partnerships
• Implement conflict prevention strategies

Requirements:
• Degree or diploma in health or social services
• Training in program planning and group facilitation
• Experience with vulnerable populations
• Knowledge of community resources
• Valid First Aid and CPR certification within 3 months
