- Provide individual supports and short-term case management to community members who are homeless or at risk of losing their housing
- Facilitate community engagement and outreach
- Create housing summits alongside community members to understand local needs, strengths, and opportunities
- Outreach and engage residents who are homeless or at risk of homelessness
- Support community members in accessing health and social services through a warm transfer process
- Develop partnerships with service agencies to ensure productive and timely access to services
- Provide short-term case management and make referrals supporting housing retention
- Compile reports on housing needs and community recommendations
- Develop and maintain a directory of available housing, supports, and services
- Explore and help develop a roommate matching program for community members
- Work in Newmarket and across York Region
Requirements
- Undergraduate degree, preferably BSW/MSW
- Minimum 1 year case management/short-term case management experience
- Minimum 1 year related experience supporting people with housing needs
- Knowledge and appreciation of housing issues in York Region
- Knowledge of legislation and regulations, including the Residential Tenancies Act
- Knowledge of eviction prevention strategies
- Knowledge of community resources and demonstrated ability to search for applicable resources/information
- Must be able to work in office and commute across York Region
- Strong interpersonal skills, with an ability to work closely with clients and partner organizations
- Problem-solving skills in identifying and referring individuals to resources
