Empower individuals facing homelessness as a Connection Coordinator with House of Friendship. This full-time role focuses on person-centered care, harm reduction, and collaboration with community resources.
As a vital member of the HART Hub program, you will engage participants in meaningful relationships while promoting their wellness and recovery goals. This position requires developing individual support plans and collaborating with community partners across Waterloo Region, primarily in Cambridge. Your expertise will help participants connect with essential resources such as housing, mental health, and addiction services.
Key Responsibilities:
• Connect with participants and foster supportive relationships
• Develop individual support plans with assessment tools
• Collaborate to achieve individual support needs
• Provide harm reduction education and primary care connections
• Engage in case conferences to support participant goals
Requirements:
• Minimum College Diploma in a related field
• 2 years of case planning/management experience
• Understanding of substance use and mental health
• Excellent interpersonal and communication skills
• Commitment to career development
