Key Responsibilities:
• Provide primary care and follow-up for vulnerable patients • Develop individualized patient care plans • Collaborate with social workers and healthcare professionals • Engage in health promotion and disease prevention • Advocate by connecting patients to community resources
Requirements: • Current NP registration with the College of Nurses of Ontario • Minimum 2 years of direct clinical experience • Robust skills in chronic disease management • Proficiency in electronic medical records • Fluency in French is an asset
