As a full time Psychogeriatric Resource Consultant, you will work directly with Long Term Care, Acute Care, and community providers to strengthen the quality of care for older adults with cognitive and behavioral health challenges. Your role will involve providing consultations, driving educational initiatives, and developing integrated care practices through collaboration with community organizations.

Key Responsibilities:
• Deliver case-based consultation to healthcare providers
• Cultivate partnerships with community health organizations




• Assist in assessing community learning needs
• Act as an active member of the SMH BSS team
• Facilitate effective service delivery enhancements

Requirements:
• Degree in a Health Care Discipline; Master’s preferable
• Three years’ experience in community healthcare or Long-Term Care Homes
• Skills in mental health assessment and care planning
• Familiarity with PIECES framework and Gentle Persuasive Approaches
• Experience in developing educational programs for staff
