St. Joseph’s Home Care is seeking a Nutrition Specialist who can provide quality dietary guidance in a part time capacity across Hamilton and surrounding areas. This role is vital for clients transitioning from hospital care.
In this position, you will perform nutritional assessments and create care plans that meet individual client needs. Working closely with healthcare teams and community resources, you will play a crucial role in promoting the nutritional health of clients, particularly those facing mental health and addiction challenges.
Key Responsibilities:
• Assess client nutritional needs and risks effectively
• Develop individualized dietary service plans
• Educate staff and clients on nutrition-related issues
• Collaborate with healthcare professionals for client care
• Advocate for clients regarding food security and resources
Requirements:
• Relevant university degree in nutrition
• Completion of dietitian internship required
• Current and unrestricted registration with the Dietitians' College
• A minimum of 3 years of experience in a similar setting
• Familiarity with geriatric and community care settings preferred
