Provide expert nutritional support as a Dietitian at St. Joseph’s Home Care in HNHB regions. This role focuses on client well-being and optimizing dietary care for various health conditions.

As a key player in the Integrated Comprehensive Care Program (ICCP), your responsibility is to ensure clients' nutritional needs are met. You'll engage with clients suffering from mental health or mobility issues, evaluate their nutrition plans, and take part in health team discussions. Your work will empower clients and staff through dietary education and robust community partnerships.

Key Responsibilities:
• Assess and monitor client nutritional status
• Evaluate dietary needs according to care plans
• Document and share insights at family care conferences




• Maintain knowledge of current nutritional research
• Implement individualized service plans for clients

Requirements:
• Bachelor’s degree in a related field required
• Successful completion of dietitian internship
• Registration with the College of Dietitians required
• 3+ years experience in a similar healthcare role preferred
• Familiarity with community care settings an asset
