As part of the Primary Care team, you will assess and develop care plans for clients with mental health and addiction challenges. Your role involves utilizing evidence-based practices and fostering collaboration among diverse healthcare professionals. Solid communication and critical thinking skills are essential in this position, as you work closely with individuals and their families.
Key Responsibilities:
• Assess client needs and create targeted care plans
• Facilitate group and individual teaching sessions
• Promote effective communication within the healthcare team
• Engage in problem-solving and critical thinking processes
• Maintain accurate and timely clinical documentation
Requirements:
• Bachelor's degree in Allied Health or related field
• Minimum two years in a health setting with complex care
• Valid BC driver’s license required
• Strong commitment to patient advocacy and social justice
• Ability to perform physical duties of the role
