As part of the Primary Care team, you will assess and develop care plans for clients with mental health and addiction challenges. Key Responsibilities:
• Assess client needs and create targeted care plans
• Facilitate group and individual teaching sessions




• Promote effective communication within the healthcare team

• Maintain accurate and timely clinical documentation

Requirements:
• Bachelor's degree in Allied Health or related field
• Minimum two years in a health setting with complex care
• Valid BC driver’s license required
• Strong commitment to patient advocacy and social justice
• Ability to perform physical duties of the role
