Wellington County seeks a dedicated Nurse Practitioner for its Wellington Terrace facility to lead and provide primary care to residents. This full-time role focuses on team-oriented care and mentoring in a supportive environment.

Reporting directly to the Director of Care, you’ll utilize your experience to enhance resident well-being. The role allows you to develop and evaluate care plans alongside an interdisciplinary team. Your leadership will guide both quality improvement initiatives and educational opportunities within the team.

Key Responsibilities:
• Deliver comprehensive primary care and assessments
• Monitor and manage medication reviews annually
• Collaborate on resident care and refer as necessary




• Lead education and research for health care practices
• Implement improvement strategies in clinical programs

Requirements:
• Four-year Nursing degree and NP graduate program
• At least three years experience in health care
• Registration in Ontario as an Extended Class RN
• Certification in Basic Cardiac Life Support
• Leadership experience in health care is an asset
