In this permanent full time position, you’ll work Monday through Friday, focusing on effective client transitions from the Toronto South Detention Centre. Your responsibilities will include assessing client needs, coordinating with referral sources, and developing tailored plans to assist with goals like obtaining shelter and healthcare. You will play a key role in fostering supportive networks for clients post-release.

Key Responsibilities:
• Participate in referral screening and eligibility determination
• Build and maintain supportive client relationships




• Conduct comprehensive needs assessments for clients
• Assist clients in creating safety and recovery plans
• Provide practical support for daily living skills and access to resources

Requirements:
• Relevant experience in case management or mental health
• Excellent interpersonal skills for client interaction
