UHN is seeking three pharmacists to provide comprehensive pharmacy services in the Inpatient Unit at Toronto Western Hospital. Collaborate with healthcare teams to ensure appropriate medication use and manage patient drug therapies effectively. You'll be integral in promoting safe medication practices while facilitating clinical education.

Key Responsibilities:
• Conduct medication reconciliation for admitted patients
• Identify and resolve drug therapy problems
• Monitor adherence to best practices and guidelines




• Provide education and support for patients and families
• Assist in clinical teaching and research activities

Requirements:
• Bachelor of Science or Doctor of Pharmacy degree needed
• Certification from Pharmacy Examination Board of Canada
• Active registration with Ontario College of Pharmacists required
• Experience in patient-centered hospital pharmacy
• Strong communication and interpersonal skills
