Join the HART Hub as a Wellness Connector focusing on Indigenous health in London, Ontario. Assist clients with housing, addiction recovery, and mental health using culturally informed strategies.
In this full-time position, you will facilitate access to services for Indigenous clients, bridging traditional healing practices with contemporary health solutions. This role emphasizes culturally respectful communication and collaboration with community resources to address clients’ unique needs. Your coordinated efforts will ensure clients receive the holistic support they deserve.
Key Responsibilities:
• Act as a liaison for Indigenous client support
• Support clients in accessing effective health services
• Advocate for shared traditional healing practices
• Develop individual plans for housing and mental health
• Build partnerships with community organizations
Requirements:
• Degree or diploma in Social Work or Indigenous Studies
• Minimum of 2 years' experience in relevant fields
• Required G2 Driver’s License for client transport
• Valid Police Vulnerable Sector Check is necessary
• Understanding of systemic challenges faced by Indigenous communities
