In this full time position, you will manage the delivery of patient support programs as a Field Nurse Case Manager. You’ll be the key liaison between healthcare providers and patients, ensuring seamless access to treatment and essential services while advocating for patient well-being throughout their healthcare journey.

Key Responsibilities:
• Facilitate patient enrollment into support programs
• Educate patients on disease management and therapies
• Support healthcare providers with funding and resource navigation




• Maintain accurate records and entry of patient information
• Conduct follow-ups to ensure continuity of care

Requirements:
• Must be a Registered Nurse with provincial accreditation
• 2-4 years of clinical experience required
• Advantageous to have Patient Support Program knowledge
• Background in oncology, gastro or rheumatology preferred
• Bilingual English and French is a plus
