As part of LMC Healthcare, Canada's largest diabetes specialist provider, your role as a Diabetes Educator aims to improve patient outcomes through education and team-oriented care. You’ll guide both individual patients and groups in self-management strategies for diabetes and related health issues. Utilizing your expertise, you will also perform patient follow-ups and leverage technology for enhanced care delivery.
Key Responsibilities:
• Educate patients about diabetes management
• Draft and implement personalized care plans
• Utilize EMR for patient record-keeping
• Instruct patients on proper insulin and SMBG techniques
• Manage follow-up consultations through diverse mediums
Requirements:
• Registered Dietitian license in Ontario
• Teaching experience with individuals and groups
• Pursuing or attained CDE credential
• Current driver's license and personal vehicle
• Experience in motivational interviewing preferred
