This Casual role within the Neuro Rehabilitation Program supports adults recovering from neurological conditions like strokes. You will work closely with the Community Care Coordinator to facilitate effective discharge plans and ensure clients transition smoothly back to their communities. Communication with clients and families is essential for monitoring progress and outcomes.

Key Responsibilities:
• Support clients in navigating discharge processes
• Address social and financial issues related to care
• Report client progress to families
• Collaborate in an interdisciplinary care team
• Participate in client assessments and planning

Requirements:
• Degree in Social Work or equivalent
• Alberta Association of Social Workers registered
• 2 years minimum experience in relevant roles
• Experience with gerontology preferred
• Current Driver’s License
