As an Integrated Behavioral Health Specialist, you will work directly under the Chronic Care Management Manager, delivering key psychosocial assessments and community resource referrals. This full-time position emphasizes independent work, collaboration with care teams, and the ability to engage effectively with patients and families. Exceptional communication skills are crucial for facilitating continuity of care and achieving optimal outcomes.
Key Responsibilities:
• Perform comprehensive psychosocial evaluations for patient needs
• Implement brief solution-oriented interventions and therapies
• Facilitate patient and family education and advocacy efforts
• Ensure document accuracy in medical records and communications
• Monitor social determinants influencing patient care
Requirements:
• Master’s Degree in Social Work mandatory
• Active LSCSW license in Kansas or Missouri required
• At least one year of relevant experience preferred
• Strong organizational and team communication skills essential
• Familiarity with community services is key
