Key Responsibilities:
• Assess and identify care needs for patients and families
• Develop individualized care plans with input from patients




• Coordinate community resources and healthcare services
• Communicate effectively with multidisciplinary teams
• Oversee patient discharges and service adjustments

Requirements:
• Degree in nursing, physiotherapy, or related discipline
• 2 years of experience in relevant fields required
• Bilingual proficiency in English and French mandatory
• Current Vulnerable Sector Check needed
• Evidence of COVID-19 vaccination necessary
