This part time position involves creating and implementing care plans tailored to client needs, and acting as the main point of contact for your assigned caseload. You'll work intensively with the healthcare team to support clients through their care journey and integrate local resources.

Key Responsibilities:
• Perform comprehensive assessments and develop individualized care plans
• Act as primary care provider and lead for clientele
• Collaborate with community services for seamless client transition

Requirements:
• Certificate of Competence from the College of Nurses of Ontario
• Commitment to 2-3 working days per week and on-call rotation
• Must clear criminal and background health checks
• CPR certification required upon hiring
• Physical fitness for demanding nursing tasks
