In this role, you will work with patients to assess their needs and develop personalized care plans, facilitating their journey to better health through effective communication and resource navigation. Collaborating with an interdisciplinary care team, you will promote diabetes self-management and enhance community functioning for your clients.
Key Responsibilities:
• Assess and support the psychosocial needs of diabetes clients
• Create individualized care plans with interdisciplinary teams
• Provide counseling and link patients to community services
• Navigate healthcare and social assistance resources
• Maintain accurate documentation and reports
Requirements:
• Post-secondary education in Social Services or equivalent
• Registration with OCSWSSW or eligibility
• Commitment to obtaining Canadian Diabetes Educator Certification
• Excellent problem-solving and interpersonal skills
• Experience in a healthcare setting is preferred
