Contribute to client care as an Onsite Case Manager at LOFT Community Services in Bradford, ON. This contract role prioritizes coaching clients facing mental health and addiction challenges while enhancing their independence.

In this 6-month Case Manager position, you will engage directly with clients to assess their needs and facilitate access to various support services. Your role will involve practical assistance, crisis intervention, and collaboration with other professionals to promote well-being. Emphasizing dignity and respect, you will guide clients toward achieving their recovery goals and enhancing their quality of life.

Key Responsibilities:
• Develop recovery and support plans through assessments
• Assist clients in learning vital daily living skills




• Coordinate care with medical and social services
• Represent the program professionally in community settings
• Support social activities to diminish isolation

Requirements:
• Degree in Social Services or Human Services
• Minimum 3 years' experience working with relevant populations
• Familiarity with challenges related to mental health and addictions
• Proficient in crisis intervention and advocacy
• Robust teamwork and documentation skills
