Support the 2SLGBTQ+ community in Calgary as a dedicated Case Manager. Use your advocacy skills and connections to empower clients throughout their resettlement experiences.
The Centre for Newcomers Society of Calgary invites applications for a passionate Case Manager. This role entails assessing clients’ distinct needs and delivering tailored support through effective case management. Your knowledge of HIV/AIDS issues and commitment to LGBTQ+ rights will guide your approach in enhancing client services. Engage actively with community networks to ensure comprehensive care.
Key Responsibilities:
• Full cycle case management from assessment to evaluation
• Conduct assessments based on social determinants of health
• Develop and continually evaluate service plans for clients
• Facilitate educational and support resources for staff
• Examine and implement changes in current processes
Requirements:
• Diploma or degree in social services; RSW preferred
• At least 2 years' case management experience
• Proven track record with Queer/Trans clients
• Understanding of LGBTQ+ immigration issues
• Involvement in local Queer/Trans community
