In the Case Manager position, you will provide essential support for women affected by various challenges, including homelessness and abuse. Key Responsibilities:
• Conduct thorough intake assessments to identify client needs
• Implement and regularly update individualized case plans
• Monitor client progress and adjust plans as necessary




• Provide advocacy and navigation support for client resources
• Facilitate safety and risk assessments for clients

Requirements:
• Bachelor’s degree in social work, counselling, or related
• At least 2 years experience with complex clients
• CPR and First Aid Certification required at start
• Registration with a relevant skilled association needed
• Strong communication and relationship-building skills
