In the Case Manager position, you will provide essential support for women affected by various challenges, including homelessness and abuse. You will develop individualized plans, assess risks, and coordinate community resources. Collaboration with clients and partners is critical for fostering resilience and independence.
Key Responsibilities:
• Conduct thorough intake assessments to identify client needs
• Implement and regularly update individualized case plans
• Monitor client progress and adjust plans as necessary
• Provide advocacy and navigation support for client resources
• Facilitate safety and risk assessments for clients
Requirements:
• Bachelor’s degree in social work, counselling, or related
• At least 2 years experience with complex clients
• CPR and First Aid Certification required at start
• Registration with a relevant skilled association needed
• Strong communication and relationship-building skills
