The position allows you to engage in health promotion and falls prevention initiatives while supporting clients in navigating disability benefits and accommodations.

Key Responsibilities:
• Carry out comprehensive client-centred assessments
• Develop and execute individualized, occupation-based intervention plans




• Perform home and community-based assessments
• Facilitate chronic disease self-management strategies
• Conduct education sessions for clients and caregivers

Requirements:
• Master’s or Bachelor’s Degree in Occupational Therapy
• Registration with the College of Occupational Therapists of Ontario
• At least three years of community-based practice
• Proven ability to work in interdisciplinary teams
• Bilingualism and proficiency in electronic medical records preferred
