In this role, you will collaborate with a dedicated team to offer assessment, treatment, and follow-up services for elderly clients with complex mental health issues. Your scope will include working with families and caregivers while building solid partnerships with community agencies to improve service delivery.
Key Responsibilities:
• Conduct psychosocial assessments and facilitate crisis interventions
• Develop tailored treatment plans for elderly clients
• Attend interdisciplinary team meetings for client coordination
• Maintain clinical records and prepare reports as required
• Advocate for clients within community health networks
Requirements:
• Master’s degree in Social Work or similar discipline
• One year of experience in mental health settings for elderly clients
• Registration with a professional health association
• Must possess a valid Class V BC Driver’s Licence
• Capable of effective communication and networking with community services
