The Occupational Therapist will work on the GAIN Community Team to assess older adults' needs, focusing on their emotional and physical well-being. You will gather data using validated tools and collaborate with an inter-skilled team to create effective care plans. Your work will significantly impact the quality of life for frail seniors.
Key Responsibilities:
• Participate in inter-professional geriatric assessments
• Conduct clinical triage for incoming referrals
• Gather data and document assessment findings
• Deliver consultation services to optimize daily living activities
• Assess seniors’ functional capabilities through various methods
Requirements:
• Degree in Occupational Therapy
• Registered with Ontario's College of Occupational Therapists
• 2-3 years’ experience in geriatric therapy
• Expertise in dementia and mental health
• Driver’s license required for home assessments
