Reporting directly to the Director of Care, the Licensed Practical Nurse will be part of an interdisciplinary team focused on delivering individualized care to residents. Key Responsibilities:
• Implement and evaluate personalized care plans for residents
• Manage medications and respond to health changes
• Collaborate with team members during transitions and assessments
• Facilitate resident education on health matters
• Document and report care updates as necessary

Requirements:
• Licensed Practical Nurse registration required
• Experience in gerontology or rehabilitation preferred
• Robust critical thinking and time management skills
• Current CPR and First Aid certification
• Willingness to complete a Vulnerable Sector Check
