Key Responsibilities:
• Implement and evaluate individualized care/service plans
• Perform accurate clinical assessments and documentation
• Manage medication administration per safety protocols
• Facilitate resident care conferences and transfers
• Respond to medical emergencies and resident concerns
Requirements:
• Licensed Practical Nurse registration required
• Experience in gerontology or long-term care preferred
• Excellent communication and coordination skills
• Current First Aid certification necessary
• Willingness to undergo Vulnerable Sector Check
