Key Responsibilities:
Conduct comprehensive patient assessments and document findings
Provide routine nursing care including vital signs and immunizations
Support chronic disease management and preventive care initiatives




Monitor patient status and communicate changes to providers
Assist with onboarding current patients ensuring smooth transitions

Requirements:
Registered Practical Nurse in good standing with College of Nurses of Ontario
Two years of clinical experience preferred in community or hospital
Current CPR certification and professional liability insurance
Experience with EMRs and clinical technology
Robust communication and interpersonal skills
