In this position, you will function as an integral member of the home care team, reporting to the Home Care Nursing Supervisor. You'll be responsible for the delivery of patient care that is culturally safe and responsive to individual needs. Key Responsibilities:
• Deliver patient care under the guidance of the Home Care Nursing Supervisor
• Collaborate with case coordinators for effective health service delivery




• Engage with clients respecting their cultural diversity
• Document care activities accurately and maintain confidentiality
• Manage travel to provide care across the community

Requirements:
• Licensed Practical Nurse and registered with CLPNM
• Minimum one year nursing experience in a related setting
• Completion of Wound Care Level 1 & 2
• Certification in Basic Life Support
• Ability to work evenings and weekends as needed
