Be a key part of The Neighborhood at Priceville as an Overnight Licensed Practical Nurse. Oversee the health of residents, delivering medication care and essential resident support.

This Wellness Nurse position requires reporting directly to the Wellness Director and involves monitoring residents’ needs through Personalized Service Plans. You'll provide leadership and clinical quality assurance, ensuring that residents receive optimal care while managing necessary documentation and communications with families and support services.

Key Responsibilities:
• Assess ongoing resident needs and services
• Document changes in clinical care accurately
• Assist with completion of monthly wellness visits
• Communicate service level changes to leadership
• Take monthly weights and vital signs for residents

Requirements:
• Licensed Practical Nurse or Registered Nurse required
• Minimum one year of experience with elderly or disabled care
• Proven leadership skills essential
• Experience in Assisted Living or Dementia preferred
• Ability to work flexible schedule including weekends
