Enhance community mental health with Fraser Health as an LPN in Mental Health Outreach. In this role, you'll provide vital nursing services to clients in their homes and community settings.
Join an interdisciplinary team focused on delivering high-quality mental health care. Your responsibilities will include assessing clients, administering medications, and adjusting care plans in response to changing client needs. You will play a key role in facilitating effective communication, ensuring patient safety, and collaborating with other health professionals.
Key Responsibilities:
• Administer prescribed medications safely and effectively
• Monitor and evaluate clients' care plans regularly
• Provide support during interdisciplinary meetings
• Document care accurately for patient safety
• Participate in quality improvement initiatives
Requirements:
• Graduation from a recognized Practical Nursing program
• Current registration with the BCCNM as a Licensed Practical Nurse
• First Aid, CPR, and Food Protected certification required
• Valid BC Driver's License and vehicle access
• Completion of MHSU Competency Education or eligibility
