As an MDS Coordinator, you will be crucial in coordinating assessments and developing individualized care plans tailored to residents’ needs. Review important documentation and represent nursing in vital meetings to advocate for resident care. Additionally, you will contribute to the team with on-call duties every fourth weekend.

Key Responsibilities:
• Manage MDS assessments and submission schedules
• Analyze condition changes through progress notes
• Coordinate care plans, updating them regularly
• Attend Medicare meetings on behalf of nursing
• Facilitate patient-centered Care Plan Conferences

Requirements:
• Valid Iowa RN license required
• MDS / RAI experience in skilled nursing preferred
• Experience with EHR/MDS software is beneficial
• Excellent organizational skills needed
