VHA Home HealthCare in Toronto (North York region) seeks a Registered Practical Nurse to deliver and coordinate home health nursing services under the Hospital Partnership Program. You will perform head-to-toe assessments and provide wound care, IV therapy, diabetes management, and other skilled tasks in clients' homes.
The role runs day shift 9:00 am–1:00 pm with bi-weekly weekend rotation, requiring robust communication, documentation, and teamwork.
