Join Trillium Health Partners as a full time Case Manager in the Mental Health Program. This role involves case management for clients with severe mental illness, focusing on recovery support within their community.

As a Case Manager, you will manage a caseload of clients experiencing severe mental illness in the Mississauga/South Etobicoke area. Your responsibilities include providing recovery-oriented support, conducting comprehensive assessments, and engaging individuals throughout their recovery process. Collaborating closely with treatment teams and clients, you will develop individualized care plans and support their journey towards independent living.

Key Responsibilities:
• Manage a caseload, providing recovery-based case management support
• Complete detailed psychosocial assessments for eligibility




• Engage clients through goal-oriented support and monitoring
• Liaise with treatment teams, including physicians
• Conduct community visits for outreach and support

Requirements:
• Bachelor’s degree or Community College diploma in health-related field
• Minimum five years' experience with clients experiencing severe mental illness
• Knowledge of recovery-oriented and trauma-informed care
• Strong organizational and communication skills
• Valid driver’s license and access to a vehicle
