In this role, you will report directly to the Director of Health and Wellness, taking responsibility for thorough documentation and resident assessments. Your expertise in gerontology and care coordination will be crucial for supporting our seniors effectively. Engage closely with families to educate them during the care process and participate in care conferences.

Key Responsibilities:
• Oversee up-to-date resident assessments and documentation
• Participate in care conferences and coordinate resident transfers
• Administer medications following regulatory standards




• Supervise medication audits and self-medication assessments
• Coordinate care plans with healthcare teams and family members

Requirements:
• Registered Nurse in the applicable Province/State
• Background in gerontology or long-term care preferred
• Excellent time management and customer focus skills
• Ability to oversee unregulated care providers' responsibilities
• Current Vulnerable Position/Sector Screening required
