In this full-time managerial role, the Community Health Integration Manager will oversee integrated programs and services to ensure high standards of client-centered care. Collaborating with various healthcare professionals, you will implement strategic plans that promote health equity and accessibility across diverse communities. Directly reporting to the VP, Integrated Care and Experience, you’ll play a crucial part in enhancing program performance and establishing effective interprofessional collaboration.

Key Responsibilities:
• Facilitate seamless integration of health services and programs
• Monitor and report on key performance indicators




• Cultivate partnerships with community and local providers
• Drive quality improvement and change management efforts
• Foster collaboration among interprofessional teams

Requirements:
• Degree in health care, public health, or social services
• Minimum of 5 years in managerial roles within health services
• Expertise in integrated care delivery
• Proven leadership in diverse and cooperative settings
• Knowledge of health equity and social determinants of health
