Lead the delivery of Community Health Services as a Personal Support Manager with CarePartners in Ontario's Quinte area. Coordinate care and support the Community Support Workers to ensure outstanding patient care.

In this full time position, your schedule includes weekdays and on-call duties in the evenings and weekends. You will develop care plans, mentor staff, and oversee the implementation of community health policies and collaboration with Ontario Health atHome. Key Responsibilities:
• Supervise and coach Community Support Workers in office
• Create comprehensive Care Plans in collaboration with Ontario Health
• Stay updated on health profession regulations
• Promote Community Health Services effectively
• Guide teams on workplace Health and Safety

Requirements:
• RN or RPN degree/diploma required
• 3-5 years of relevant healthcare experience
• CNO registration essential
• Leadership experience preferred
• Clear background and vulnerable sector check
