This full time temporary role at Toronto Rehab - University Centre seeks a creative and collaborative social worker to support patients with movement disorders. You will lead psychosocial assessments, contribute to comprehensive treatment plans, and engage with a multidisciplinary team to enhance patient experiences and outcomes.
Key Responsibilities:
• Deliver psychosocial assessments for patients with neurological disorders
• Formulate comprehensive treatment strategies within a team
• Actively participate in multi-disciplinary care coordination
• Follow up with patients via telehealth and in-person visits
• Facilitate community connections to support patient needs
Requirements:
• Master’s Degree in Social Work is needed
• Registration with Ontario College of Social Workers required
• At least four years in clinical practice
• Skills in cognitive behavioral therapy are advantageous
• Proven ability to thrive in collaborative healthcare settings
