Become a Primary Care Nurse Practitioner with the South East Grey Community Health Centre in Dundalk. This full-time role offers a 14-month contract starting end of October 2026.

As an NP in a supportive multi-disciplinary environment, you'll collaborate with physicians and health staff to provide quality care. Your experience and strong communication skills will play a pivotal role in serving vulnerable populations. We're looking for someone who thrives in adaptable settings and is committed to fostering inclusivity in healthcare practices.

Key Responsibilities:
• Engage collaboratively with various healthcare professionals
• Exercise initiative and clinical judgment in independent work
• Adjust efficiently to a dynamic healthcare landscape




• Promote inclusivity and professionalism within the team
• Implement chronic disease management strategies and programs

Requirements:
• Registered member in positive standing with the College of Nurses of Ontario
• 3-5 years of progressive experience in primary care
• Proficiency in electronic medical records and Microsoft Office
• Understanding of the Community Health Centre care model
• Experience with diverse and underserved communities
