Join the PFK Care Coordination team as a Full time RN Care Coordinator. Focus on providing quality care for children with special needs, utilizing your nursing expertise in diverse healthcare settings.

As an RN Care Coordinator at the Child Health Pavilion, you will work in collaboration with families and healthcare professionals to craft effective care plans. This position requires a proactive approach to care management and transition facilitation to ensure optimal patient outcomes. You will work primarily with children in custody, adoption, and foster care, emphasizing patient advocacy.

Key Responsibilities:
• Oversee individualized patient care plans with healthcare teams
• Ensure seamless transitions to different levels of care




• Communicate care plans to families and medical staff
• Promote ethical practices in nursing
• Manage post-discharge care transitions

Requirements:
• Bachelor’s degree in Nursing required
• Valid RN license in Ohio required
• Minimum of 3 years nursing experience
• BLS certification within 60 days of hire
• Excellent skills in patient advocacy and teamwork
