Empower individuals within the Kwanlin Dün First Nation as a Community Health Advocate. This role focuses on navigating health services and advocating for clients’ needs in a culturally safe environment.
Working from the Natsékhi Kų̀ Health Centre, you will help vulnerable individuals access comprehensive care and manage complex health conditions. Strong communication skills and a background in social work or advocacy are essential for building trust and effectively supporting clients through difficult journeys.
Key Responsibilities:
• Guide clients through intricate case management services
• Develop and implement comprehensive wellness plans
• Ensure safe access to healthcare resources
• Facilitate connections between clients and community services
• Enhance clients’ health literacy through education
Requirements:
• Bachelor of Social Work with a minimum of 2 years experience
• Familiarity with crisis intervention and motivational interviewing
• Robust relationship-building and organizational skills
• Knowledge of community resources within KDFN
• Clear criminal record check and First Aid certification
