This role requires a Registered Nurse focused on chronic disease management, specifically diabetes. You will lead educational initiatives, create individualized care plans, and support patient self-management. Your involvement in outpatient assessments and interdisciplinary collaborations will ensure the quality of diabetes care provided to patients with complex needs.

Key Responsibilities:
• Implement individualized care plans in collaboration with health professionals
• Facilitate patient education through both individual and group settings




• Document timely and precise patient observations
• Contribute to program evaluation and development
• Provide mentorship to recent nursing staff

Requirements:
• Graduation from an accredited nursing program
• Two years of clinical experience in diabetes management
• Accomplished Diabetes Educator certification
• CPR certification current
• Effective verbal and written communication skills
